YORK HOUSE
York House, located in west Rogers Park, is a 12-bed, supported, group living home for adults recovering from psychiatric illnesses. This residence provides a supportive, home environment in which individuals can pursue personal wellness and fulfillment of life goals. Services include counseling, skills training, case management, and support with medications and with other health issues. Research-based practices are employed to help individuals address and manage psychiatric issues, improve self-care skills, and establish personal goals. Peer support, friendships and family, and integration into the larger community are essential components of the Thresholds model. Our overarching goal is to help each person define and achieve personal recovery.

York House is a three-flat building in which each person has his or her own bedroom, with full access to all common areas including living rooms, recreation rooms, dining rooms, kitchens, laundry, cable television, internet, and telephones. The house is staffed 7 days a week, 24 hours a day, by Thresholds Community Support professionals, including a registered nurse. A group meal is prepared each evening. Staff-supported group outings and activities are available several times throughout the week. For some individuals, York House has been a place to prepare for living on their own; others have chosen to make it their home. Length of stay is determined by individual need.

6710 NORTH
6710 North is a clustered apartment program for young adults ages 18-30. It is located in east Rogers Park in the Loyola University neighborhood near stores and transportation. In a large, mixed-use building, each resident lives in his or her own apartment. Community support professionals are on-site 24/7 and work out of the community room and staff office. They work with the residents to develop and utilize a supportive peer community. Services include counseling, life skills training, social skills training, support with community integration, case management, and assistance with medications and with other health issues. Services are research-based and are designed to help young people learn to manage their psychiatric illnesses successfully and to experience a greater sense of personal wellness and fulfillment of life goals.

Acceptance into any Thresholds program is based upon a clinical assessment to assure that Thresholds can be of resource and is appropriate to the level of care and needs of the individual. COMMUNITY SUPPORT SERVICES
Community Support services are available for individuals living in their own homes, with family, or in nursing homes. Counseling, skills training, case management, and support with medications and with other health issues are key components.

6710 North - $4,250/month
Outreach - $100/hour plus travel
