Thresholds understands the complicated process of re-entering society after being incarcerated. For many, the difficulty in finding community resources can seem hopeless and often results in a vicious cycle of repeat imprisonment. Thresholds stops this revolving door. Our programs are designed to address the variety of issues members face and assist them in regaining full control over their lives.
Our assertive community treatment approach is funded by the Illinois Office of Mental Health and works in partnership with the court system. Thresholds works with chronic non-violent detainees who have severe and persistent mental illness. Most of these people do not belong in jail; they belong in a supportive, recovery network focused on their specific needs. Thresholds provides a support system – not the court system – for people with mental illness.
Thresholds' services cost about $26 a day compared with $70 a day at Cook County Jail.
Thresholds' personal and intensive case management program employs a team of outreach workers for home visits 7 days a week (including holidays) for as long as the member needs our help. If a member is homeless, our workers will scour the streets to find and assist him or her. This type of commitment has demonstrated an 80% reduction in the need for hospitalization amongst those managing the most severe mental illnesses. |
