Most individuals we serve (“members”) have been diagnosed with a serious mental illness. Diagnoses include schizophrenia, bipolar disorder, major depression, and severe post-traumatic stress disorder.
At intake, 90% of our members are unemployed and living well below the National Poverty Level, approximately 50% have co-occurring substance abuse conditions, and most have at least one advanced physical health problems such as diabetes and heart disease.
Given such challenges, a comprehensive care model built of a variety of programs and services is essential in order to create a solid foundation of recovery. Thresholds believes in a respectful, collaborative approach, where staff and members work together to design an individualized course of treatment. Our prevailing goal is to help our members experience lives of greater independence and fulfillment.
Thresholds members are active in leadership throughout the organization, including a members council, program leadership, and advocacy. We believe that consumer input is critical to all program development and execution.
