Private Access Services
Thresholds is now able to offer its services on a private pay basis in certain locations. We can accept insurance for eligible services in our Outreach program, including psychiatry and in-home counseling and psychotherapy. We offer private pay schedules to meet the needs of the member or payer. We also have a private pay group home.
Community Based Treatment and Support
When you or someone you care about is struggling with mental health or emotional issues, the right care and support can make a tremendous difference. Although there is still much to be learned and understood about how the brain functions and about the conditions that affect the brain, we know that a holistic approach is the key to recovery. The Thresholds Private Access Program’s approach is one that recognizes and addresses the individual’s total health needs, self chosen goals and dreams, and personal values and preferences.
We work with individuals who are experiencing health conditions including bipolar disorder, depression, anxiety, agoraphobia, schizophrenia, personality disorder, complicating substance abuse and dependence, OCD, and trauma. We work with persons in their homes and in their communities, whether living at home with family, independently, or in nursing homes.
The Thresholds Private Access Program, developed from Thresholds’ almost 60 year foundation of excellent service and leadership in mental health care, uses evidenced-based treatment modalities that provide practical support and assistance with daily living, in the context of a therapeutic partnership with the individual.
The Thresholds Private Access Program collaborates with third party payers and insurance companies to provide the most appropriate services available under the individual’s benefit plans. Our intensive community-based services for high utilizers insure improved HEDIS scores and lower readmission rates to more acute levels of care. Private schedules can be developed to meet the needs of the member.
We believe in the whole person’s recovery, and we offer services to support the individual’s goals in recovery. Services include:

“Outreach” – Community-Based and In-home Treatment and Support
Outreach services are available for individuals living in their own homes, with family, or in nursing homes. Counseling, skills training, case management, psycho-education, and support with medications and with other health issues are key components. Community Support services focus on individual goals, building life skills, and community integration. Some insurance coverage may apply.
The group home is located in West Rogers Park. It has 15 bedrooms for 15 adults. It serves both permanent residents as well as individuals transitioning to independent living. Services include counseling, care coordination, psycho-education, enrichment and recreation activities, assistance with medication, and others, all with a focus on personal goals, mental and physical health, life skills, and community integration. Meals and amenities are included. The house is staffed 7 days a week, 23 hours a day, by our Community Support Specialists, including a registered nurse. Please note: The group home services are generally not covered by insurance plans, and are currently $3850 per month.
