Thresholds Partners with Wellframe to Empower Veterans to Better Self-Manage Care with Mobile Technology

Leading community-based mental health agency rolls out Mobile-Enabled Platform to Veteran populations in order to enhance care delivered and improve long-term health outcomes

Chicago, IL – Wellframe, a Boston-based company that enables health plans and healthcare providers to amplify existing care resources to improve patient engagement and better manage care, announced today that the organization is partnering with Thresholds to provide its members in the Thresholds Veterans Project with Wellframe’s mobile technology to help them better navigate the healthcare system and gain control over their own care. Thresholds is a leading provider of recovery services for persons with mental illnesses in the Chicago area, and delivers evidence-based practices that include homeless outreach, housing services, supported employment, peer-driven supports, substance abuse treatment, integrated physical and mental health services, and trauma-based therapies. Their veterans-specific program, the Thresholds Veterans Project, is a leader in community-based social services tailored specifically for veterans facing mental health challenges.

These more meaningful interactions, combined with the valuable resources and tools available through Wellframe’s mobile application, enable veteran members to overcome barriers to care, leading to long-term recovery.
