Emerge and MindStrong Programs Improve Lives of Young Adults Living With Mental Health Conditions

In the last year, Thresholds’ Emerge and MindStrong Programs have made strides in reducing psychiatrical hospitalization among young adults living with mental health conditions.

Both programs are dedicated to assisting youth and young adults from all backgrounds living with conditions that include schizophrenia spectrum disorder, bipolar disorder, depressive disorders, anxiety disorders and trauma-related disorders. The Emerge Community Support Teams provide comprehensive behavioral health services to young adults ages 18-26, helping them meet their personal life goals and manage their mental health conditions.

MindStrong is a Coordinated Specialty Care (CSC) program, focusing on early intervention for Clinical High Risk for psychosis and first episode of psychosis. The program assists individuals ages 14-40 who are experiencing a recent onset of psychosis and people ages 13-25 who are at Clinical High Risk for psychosis, providing both individual and family services.

Emerge Clients 2021

Prior to enrollment: 70% experienced a psychiatric hospitalization; 71% experienced trauma

Program Exit:

  • 100% avoided or decreased days psychiatrically hospitalized
  • 78% were working and/or in school
  • 100% of planned exits were connected to a clinically appropriate level of care

MindStrong Clients 2021

Prior to enrollment: 77% experienced a psychiatric hospitalization; 72% experienced trauma

Program Exit:

  • 96% avoided or decreased days psychiatrically hospitalized
  • 83% were working and/or in school
  • 85% of planned exits were connected to a clinically appropriate level of care
